Can your patient benefit from our services?
MHC welcomes referrals from community practices. We will work collaboratively with you to ensure your patient receives the comprehensive services they need and provide great communication with your office along the way.
Please call our Intake line at 615-743-1555 or 866-816-0433 (toll free) where we are happy to answer any questions and schedule an appointment for your patient.
If your patient is an adult, we will need to get their approval. We are happy to call them if they have already left your office. If a child is being referred, you can go ahead and make the referral.

We also have a simple “physician’s online referral system.” Please fill out one of our online forms below to initiate a referral. All information is encrypted for confidentiality.
We welcome walk-ins at all locations during office hours.
