Homeless Outreach Services

Our Homeless Outreach team helps connect individuals experiencing chronic homelessness to treatment and resources. Once enrolled, our Homeless Services team works closely with these individuals to access and transition to affordable, sustainable housing, in addition to assisting with linking and referring to resources and ongoing care to support their overall well-being.

MHC is part of the Projects for Assistance in Transition from Homelessness (PATH) through the Tennessee Department of Mental Health and Substance Abuse Services. The PATH program assists individuals experiencing homelessness who have mental illness or co-occurring disorders. MHC’s Homeless Outreach team helps connect individuals experiencing chronic homelessness to treatment and resources. Once enrolled, our Homeless Services team works closely with these individuals to access and transition to affordable, sustainable housing, in addition to assisting with linking and referring to resources and ongoing care to support their overall well-being. MHC provides these specialized services for individuals experiencing homelessness in the following counties:

  • Davidson
  • Sumner
  • Montgomery
